Can You Bring Tramadol to Japan? Rules for Travellers
Can you bring tramadol to Japan? In most cases yes, for your own treatment and within the limits. Tramadol is an opioid, but Japan's Narcotics Control Department states that it is not a narcotic (麻薬ではない). That makes the rules very different from those for codeine or oxycodone.
Quick answer
For tramadol (tramadol hydrochloride tablets or capsules) that you carry yourself for your own treatment: the NCD says tramadol is not a narcotic, so no narcotic permission is needed. The ordinary prescription rule applies: up to one month's supply by the dosage on the label. For more than one month, apply for Import Confirmation before you enter Japan. Syringes alone also need Import Confirmation. Codeine and oxycodone are different: they are narcotics that need advance permission. Check your exact product and quantity, because the rules can change.
Not sure whether your supply is within one month? Check the ingredient, strength, daily dose and total amount before you travel.

Can you bring tramadol to Japan? Why it is not a narcotic
Tramadol is an opioid pain medicine, so many travellers assume Japan treats it like morphine or codeine. It does not. The Narcotics Control Department (NCD) says in its English guidance for carrying controlled drugs: “TORAMADOL is not a narcotic.” Its examples of narcotics are morphine, fentanyl, oxycodone, codeine and tapentadol.
Tramadol トラマドール also does not appear in the NCD psychotropic quantity table, which we checked on 4 October 2026. As a result, the narcotic advance permission and the psychotropic table amounts do not apply to it. The MHLW Q&A for importers then governs: general prescription medicine for personal use, which is not a narcotic or a banned drug, may be brought in up to one month's supply.
Not a narcotic does not mean unrestricted. The Japanese label for tramadol hydrochloride OD tablets lists it as a potent drug and prescription medicine 劇薬・処方箋医薬品. That is why the one-month rule matters.
Three checks before you bring tramadol to Japan
Not a narcotic
The NCD says tramadol is not a narcotic, so the narcotic permission application does not apply.
One month's supply
MHLW allows prescription medicines up to one month by the dosage on the label. Above that, Import Confirmation is needed before you enter.
Syringes need confirmation
The MHLW Q&A says syringes alone need Import Confirmation as medical devices, even for one piece.
The checks are independent, so run all three. Our guide to Import Confirmation (yakkan shoumei) explains the process for supplies above one month. In addition, MHLW's personal-import page states the one-month rule for prescription medicines and potent or toxic drugs.
Step by step: how to bring tramadol into Japan
- Read your label. Note the active ingredient (tramadol hydrochloride), the strength, whether it is combined with another ingredient such as paracetamol, and the daily dose.
- Count the days. Multiply the label dose by the days you will carry. MHLW measures one month by the dosage and administration.
- Stay at or under one month if you can. If you need more, apply for Import Confirmation through the Regional Bureau of Health and Welfare before you enter Japan.
- Pack it yourself. Keep the medicine in its original pharmacy-labelled packaging in your carry-on, with your prescription or a prescriber's letter.
- Check the other medicines. If you also take codeine, oxycodone or another narcotic, those need NCD advance permission two weeks before travel.
Tramadol quantity limit: how to work out one month
There is no fixed gram figure for tramadol, because it is not on the psychotropic table. Instead, the limit is a time limit. The doses below are for arithmetic only and are not dosing advice, so use the strength and daily dose on your own pharmacy label.
| Example label dose | Days you will be in Japan | Tablets needed | Within one month? |
|---|---|---|---|
| 1 tablet three times a day | 14 | 42 | Yes |
| 1 tablet three times a day | 30 | 90 | Yes |
| 1 tablet three times a day | 45 | 135 | No: Import Confirmation first |
| 2 tablets twice a day | 60 | 240 | No: Import Confirmation first |
Count the supply, not just the trip. A short trip with a large stock can still exceed one month by the label dose, so carry what you need rather than a full pharmacy supply.
What to carry when you bring tramadol to Japan
- Medicine in its original pharmacy-labelled packaging, showing your name and the dose
- A prescription or medication list showing the active ingredient (tramadol), strength and daily dose
- Your supply worked out in days by the label dose, at or below one month unless you hold an Import Confirmation
- An Import Confirmation (yunyu kakunin-sho) if you carry more than one month's supply
The MHLW Q&A sets no certificate requirement for general prescription medicine within one month, but carrying a prescriber's letter is still sensible. If your documents are in a language other than English or Japanese, a medical translation may help the officer read them.
Tramadol vs codeine and other opioids at the Japanese border
Japan's rule follows the active ingredient, not the brand or the fact that it is an opioid. The NCD names the categories below, so check each ingredient in your bag separately.
| Ingredient | Category (NCD) | What travellers need |
|---|---|---|
| Tramadol | Not a narcotic | Ordinary prescription rules: up to one month's supply, own use; Import Confirmation above that. |
| Buprenorphine | Psychotropic | No NCD permission; quantity limits and documents apply (see the NCD psychotropic table). |
| Codeine, oxycodone, morphine, fentanyl, tapentadol | Narcotics | Permission from the Regional Bureau of Health and Welfare before entering Japan, applied for two weeks before arrival or departure. |
The narcotic permission process is set out on the NCD Japanese guidance page. See our overview of controlled medicines in Japan for each category. This article covers tramadol only; other ingredients need their own check.
Tramadol in Japan: what the Japanese label says
If you plan to stay, it helps to know how tramadol is positioned locally. The PMDA package insert for tramadol hydrochloride OD tablets (revised June 2024) lists these points:
- The indication is pain relief in conditions that are hard to treat with non-opioid analgesics, such as cancer pain and chronic pain.
- It is contraindicated in children under 12.
- It warns that continued use may cause drug dependence, so doctors prescribe carefully.
As a result, you should not assume a Japanese doctor can simply continue your current tramadol regimen. A doctor in Japan decides what to prescribe, and the indications may differ from those in your home country.
What to do if tramadol can't come in
If your supply exceeds one month, or you cannot complete the paperwork in time, you have several options. Discuss them with your prescriber before you travel.
- Bring one month or less. Within one month, no Import Confirmation is needed under the MHLW rule.
- Apply for Import Confirmation. For more than one month, apply through the Regional Bureau of Health and Welfare before you enter Japan. See our Import Confirmation guide.
- Arrange care in Japan. Find a doctor who can communicate in English, or book an interpreter for the visit, and bring an English letter from your prescriber.
Do not stop or change your pain medicine because of travel paperwork. Discuss any gap, change or timing with your prescriber well before you travel.

Frequently asked questions
Can you bring tramadol to Japan?
Usually yes, for your own treatment, within the limits. Japan's Narcotics Control Department (NCD) states that tramadol is not a narcotic, so the narcotic permission procedure does not apply. Tramadol is then treated like other prescription medicines: up to one month's supply by the dosage on the label, for personal use. For more than one month's supply, apply for Import Confirmation before you enter Japan.
Is tramadol a narcotic in Japan?
No. The NCD English guidance says tramadol is not a narcotic. Its examples of narcotics include morphine, fentanyl, oxycodone, codeine and tapentadol. Tramadol also does not appear in the NCD psychotropic quantity table.
Is tramadol banned in Japan?
No. Tramadol is sold in Japan as a prescription medicine. The Japanese package insert for tramadol hydrochloride OD tablets lists it as a potent drug (劇薬) and a prescription medicine (処方箋医薬品). Banned drugs for individuals are different, for example methamphetamine and amphetamine.
How much tramadol can I bring to Japan?
MHLW's personal-import page allows up to one month's supply, calculated from the dosage and administration, for prescription medicines. As an illustration only, a label of one tablet three times a day gives 90 tablets for 30 days. Use the dosage printed on your own pharmacy label. More than one month needs Import Confirmation.
Can I bring codeine or oxycodone instead?
These are treated differently. The NCD lists codeine and oxycodone as narcotics, which need permission from the Regional Bureau of Health and Welfare before you enter Japan, applied for two weeks before arrival. Do not assume the tramadol rule applies to them.
Can I post tramadol to Japan?
The MHLW Q&A says general prescription medicines of up to one month's supply may be sent if the other conditions are met, but narcotics, psychotropics and stimulants' raw materials may not be sent. Because conditions apply, carrying your own supply under the rules in this guide is the simpler route.
Do I need a doctor certificate to bring tramadol to Japan?
The NCD certificate requirement applies to psychotropics above the table amount or in injectable form, and tramadol is not on that table. The conditions the MHLW Q&A lists for general prescription medicines within one month do not include a certificate. We still suggest carrying your prescription or a letter from your prescriber, showing the active ingredient, strength and daily dose, so an officer can read it.
Can MedicTranslate check whether my tramadol quantity is within the limits?
Yes. Our from US$59 medication assessment reviews your ingredient, strength, dosage form and quantity against the published NCD and MHLW guidance and explains the likely next step. We do not give medical advice or guarantee customs approval; the final decision rests with the Japanese authorities.
